Questionnaire Instructions

Enter your name, company name, and position in the spaces provided. You will be shown 24 sets of 4 sentences. As you complete each 4 sentence set in the questionnaire, you must rank order each of the 4 items within a set, with 1 being the sentence most like you, 2 the sentence second most like you, 3 the sentence third most like you and 4 the sentence least like you. Repeat the process for each of the 24 sets of sentences. See the example below. When you respond to each of the 24 sets of sentences, keep your focus on the sentences in each of the sets that apply to you in the workplace. The questionnaire can be completed in 12-20 minutes.
